The state annual qualifying patient application fee is $100. However, a reduced rate of $50 is offered for qualifying patients that are on federal Supplemental Security Disability Income (SSDI) and Social Security Income (SSI).
A copy of a letter or documentations from the Social Security Administration certifying and stating the amount of the monthly Social Security and SSI disability benefits to be received by the qualifying patient during the current year of application and a copy of DD214 for veterans must be submitted as a proof of participation.
